Indication/Usage:

Tricor has been approved for the treatment of various cardiovascular indications as follows:

  • Primary hypercholesterolemia or mixed dyslipidemia: Tricor has been used as adjunctive therapy for the treatment of elevated blood levels of low-density lipoprotein, total cholesterol, triglycerides (fatty acids) and apolipoprotein B, and decreased high-density lipoprotein in adult patients with primary hypercholesterolemia or mixed dyslipidemia.
  • Hypertriglyceridemia: Tricor has been used as adjunctive therapy for the treatment of severe hypertriglyceridemia in adult patients. Tricor has also shown beneficial effects on the insulin resistance featured by the metabolic syndrome.
  • Tricor demonstrates uricosuric properties and can therefore be used for gout management.
  • By lowering the amount of fibrinogen in the blood, Tricor exhibits blood thinning properties
  • Dose, Administration and Dosage Forms: Tricor is available in orally administered tablet formulation in 48mg (yellow color) and 145mg (white color) strengths. It is usually started as 145mg once daily for the treatment of primary hypercholesterolemia or mixed dyslipidemia. Tricor should be started with the 48mg tablets once daily for the treatment of severe hypertriglyceridemia and should not exceed 145mg once daily. For patients with renal impairment, Tricor should be started at 48mg once daily. Dose selection for geriatric patients must be done on the basis of the patient's renal functioning. Tricor can be taken with or without meals and should be stored at room temperature between 15 C to 30 C (59 F to 86 F).

Mechanism of Action:

Tricor, which contains fenofibrate as an active ingredient, activates the peroxisome proliferator- activated receptor alpha. Fenofibrate activates lipoprotein lipase and reduces apoprotein C-III production which results in increased lipolysis and elimination of triglyceride-rich components from the plasma. Fenofibrate also acts on the liver, causing it to produce fewer triglycerides and increase their elimination rate. Decreased triglycerides lead to the alteration of size and composition of LDL from small, dense particles to large particles and the larger the particle, the greater the affinity towards cholesterol receptors and the faster the catabolization rate. This entire process leads to decreased bad cholesterol and triglycerides and increased good cholesterol.

Side-effects and Safety Profile:

Patients experiencing severe allergic reactions to Tricor, such as hives, dyspnea, swelling of the face, lips, tongue, or throat, should seek immediate emergency medical attention.

Tricor-associated side-effects reported during clinical trials include abdominal pain, abnormal liver function tests, abnormal weakness, back pain, bloating, constipation, fever, flatulence, flu related symptoms, headache, increased ALT, increased AST, increased CPK, indigestion, joint pain, nausea, rashes, respiratory disorders, rhinitis, etc.

Various adverse events reported during post-marketing studies of Tricor include: acute renal failure, anemia, arthralgia, asthenia, cirrhosis, decreased hematocrit, decreased hemoglobin, decreased WBC, hepatitis, muscle spasms, myalgia, rhabdomyolysis, and pancreatitis.

Warnings and Contraindications:

  • Tricor is contraindicated in patients with various disorders such as severe renal impairment, current liver disease, including primary biliary cirrhosis, and unexplained persistent liver function abnormalities, preexisting gallbladder disorder and in patients who are currently on dialysis.
  • Tricor is contraindicated in patients with known allergic reactions to fenofibrate or fenofibric acid.
  • Skeletal muscle: Tricor is reported to increase risks for myopathy and have been associated with rhabdomyolysis.
  • Liver function: Tricor in 96 mg to 145 mg daily dosages has been associated with increased serum transaminases (SGOT and SGPT).
  • Serum creatinine: Tricor has been associated with increased serum creatinine levels. These levels return to baseline after fenofibrate withdrawal.
  • Tricor is classified as a pregnancy category C drug, which means that it has not been studied in pregnant women; therefore, it should only be prescribed to pregnant women if the potential benefit justifies the potential risk to the fetus.
  • Nursing women: Tricor must not be prescribed to nursing women. Patients with a justifiable need for Tricor should consider discontinuing breastfeeding.
